Tattoo Contract US Sample Agreement
Parties:
Tattoo Artist: Jane Doe Artistry
Address: 123 Ink Lane, Los Angeles, CA 90001
Client: John Smith

Design and Placement:
The tattoo design selected is a customized dragon, to be placed on the client’s right forearm, as described in prior consultations.
Payment Terms:
The total cost for the tattoo shall be $500, payable in full prior to the appointment, unless otherwise agreed in writing.
Artist Responsibilities:
The artist shall perform the tattooing in accordance with professional standards, ensuring hygiene and safety protocols are followed.
Client Responsibilities:
The client agrees to follow all pre- and post-care instructions provided by the artist and inform of any health issues that may affect the procedure.
Liability & Release:
The client understands and accepts the risks associated with tattooing and releases the artist from liability for any adverse reactions, provided the artist adheres to professional standards.
Governing Law:
This agreement shall be governed by the laws of the State of California. Any disputes shall be resolved within the courts of Los Angeles County.
Additional Provisions:
- The client confirms they are of legal age and have no medical conditions that contraindicate tattooing.
